New Patients

To schedule an appointment, please call our office at 312-235-6773, use our Contact Us form, or email us at [email protected], so we may promptly schedule your visit. Before coming in for your first visit, we ask you take a moment to fill out our new patient forms located here. We will have these available at our office for your convenience, but it will expedite your visit to preemptively download them.

Initial Consultation

After your consultation, one of our Elite doctors will perform a complete functional examination, testing your reflexes, your ability to move through functional movement patterns as well as other standard neurological, orthopedic, postural, and physical examinations. The provider will communicate with you throughout this process so you have a thorough understanding of what we are doing, what the tests are trying to identify, and what information we are gathering from them.  If x-ray, MRI, etc. is deemed necessary, we have a trusted referral base for nearby locations. Nothing will be done in our office without your consent.

Report of Findings

Once all the information and examinations have been performed, your doctor or physical therapist will give you a report of findings to answer three very important questions:

  1. What is our working diagnosis (based on all available information, what do our providers think is going on).
  2. What are our recommendations for treatment?

If you have a condition that requires care with other providers, the doctor will discuss the best options for this as well. If we believe we can help you, we will recommend a treatment and wellness program to get you on the path towards your Elite Health.

Treatment

Patients will generally be provided with their first treatment at the conclusion of the initial appointment. This may include spinal adjustments, physio-therapies, and/or soft tissue therapies. Treatment is interactive with you so you can express any concerns you have about the style of adjustment or treatment you will be most comfortable with.

Wellness Program

Prior to leaving, patients will be instructed on a wellness program to be conducted at home. If you are in pain when you first come into our office this may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success. Every body is unique and different and therefore everyone requires a unique and customized wellness plan. The foundation of our overall wellness lifestyle is maintaining healthy posture and good spinal alignment, eating a healthy diet, taking supplements, keeping your weight under control and stress relief.

If you are wondering if your insurance plan will help toward the total cost of care, our front office staff will be more than happy to check on your benefits before you arrive in our office. We work with many of the major carriers, including but not limited to:

  • Blue Cross Blue Shield PPO
  • United Healthcare PPO
  • Aetna PPO
  • Medicare

The front office can also give you their best estimate in terms of cost per treatment once the doctor or therapist has made their recommendations.  Please note that each visit may vary slightly depending on the services provided for that visit.
