Book an AppointmentBefore your appointment
Complete your online intake forms before you arrive for your first appointment. These forms collect vital information regarding your medical history, the reason for your visit, and help gather your insurance and payment information. Having this filled out prior to your initial visit will give the doctor time to review your history and it will give the office time to verify your insurance benefits.
A provider referral is not required, but if you have one, please feel free to bring it with you.
Bring any relevant medical records, including any imaging studies that have been performed, if you have them.
What to expect at your first visit
New patient visits usually take 30-60 minutes. Our experts will perform a thorough evaluation that may require multiple tests and movements. Please dress in clothing that will allow you to move comfortably and that will allow the provider to evaluate the necessary areas.
Once your provider has completed their evaluation, they will make recommendations regarding their diagnosis and if necessary they will provide treatment or recommendations based on their findings.
Our experts work with many local providers and coordinate care across services. If they feel you need a referral, they will refer to the appropriate provider in order to best manage your care.
